DD just recently got out of her brace and the doctor said she was healed based on her latest scan. We start PT this week.

When we get to the point of her being able to throw should we start including underhand throwing as well or maybe some of the initial IR drills or maybe something else.

Im just talking about throwing and not full pitching leap and drag.

Once you get clearance, let her pain be the guide. We started with the i/r drills that didn't require lower half at all. No torque at all, and especially no hyper extension. If she is comfortable with a small movement, let her gradually make it bigger over the course of weeks, not days. A big part of recovery is mental, too. She might be afraid that she'll hurt herself again and have to start over.

Glad to hear that she's on her way to returning to sport!
