Yes the CCA manual has both 'Sell Out' and 'Punch Out' mechanics for close outs.

There is not a signal to indicate ball four (unless the batter is hit by the pitch and it was not in the batters box)

This is interesting, I feel like most umpires give the open-handed gesture to 1B after ball four. DD’s pitching coach always tells her to make her best pitch with two strikes, because umpires love to punch batters out

At a HS game last week, DD played a team full of girls who had clearly never been on a softball field. The umpire had to continually prompt them to go back to the dugout after strike three. “Okay, you’re done young lady. You can go sit down now.”
