There definitely was "starters" in the rule. We used to get the occasional umpire or coach who would question both the re-entering the DH and the fielder she was hitting for.
The explanation always came fact to "All starters may re-enter one and 10 players started, so all of them can re-enter."
A lot of coaches were somehow convinced that because a girl fielded but didn't bat she couldn't re-enter.
I think my last year coaching high school was 96. Hmmmm, now I'm intrigued.
