I'll leave the expert analysis to those who are much better at breaking down swings. What I see is it looks like the ball may be teed too far out in front of her. If she's practicing this way, and then replicating it in a game, she may be rolling over the ball. Try teeing it back further, off the front foot for down the middle, and even further back for outside pitches. Have her practice "letting the ball travel" into the zone.
We have worked with her hands positioning, Not sure how/why, but they always end up high again. No disagreement the with swing path. It is something she has been working on a bunch. However, she has been stuck swinging down/slicing the ball for a while. I think her hands need to pivot earlier and be more engaged from the start of the swing.
