Your DD does not understand the difference between "pitching in a game" and "pitching in practice." It might help to explain the difference.
Practice is practice...she is supposed to be working on something when she is practicing. She is trying to perfect her motion. Pitching in a game is different. Her focus is on getting batters out, not on worrying about her motion.
You want her to practice her "in game" routine. Her pre-pitch in game routine should be something like:
1) Receive the ball.
2) Look at outfielders and show them how many out. (That is, the pitcher should focus on a far away object.)
3) Get on rubber.
4) Get sign.
5) Cleansing breath.
5) Go.
