As simple as it sounds, just paying attention make a big difference. See a lot of younger pitchers not even thinking about the runner. Past ball, pitcher need to be moving. A lot has to go right to get the girl at home. I agree with suggestions above, probably more to do with catchers abilities getting ball back to a waiting pitcher.

Just like every other defensive play in softball, they need to practice it. We always taught the pitcher to be in fair territory, slightly up the 3rd base line so that a) she didn't get slid into and b) when she applied the tag it was before the base runner's foot could reach home plate. Many times in live games, the throw is on time but since the pitcher is right over home plate, the tag is applied too late. The other point is that the pitcher is the eyes for the catcher to help them find the passed ball/wild pitch. Communication is critical. Covering home plate drills should be practiced regularly.
