Have seen it defined many ways. Not sure there is a correct answer. One that makes most sense to me is a pitch with an entry point toward the rear of the plate either along the side edge or rear "V" of the plate. Hence the description of back door. In this definition inside/outside orientation is moot, but thrown as described it is nasty.
