Help with stepping out

I've attached a vid to look at.
She has told me she is NOT scared at all of the pitchers, But when she gets ready to swing, it looks like she is ALWAYS using her body in a way that she's trying to generate power that it's pulling her front leg completely out of way, really pulling it out.
Believe it or not, She has been consistently hitting the ball this way for some reason, not sure how.
What can I do to help her stay PLANTED in the box?

Yes, She does it with all pitches.

Yes, and with a pitching machine as well.

When I ask her to blast the ball during T work, She does it the worst.

Put a 6 foot 2x4 down behind her and touching her heels when she hits off a tee and in the cage.

If she does it on tee work, I'd think it's a mechanical flaw and not an issue of being afraid.

Yes, I agree, it seems to be a mechanical flaw for sure, almost like she's using sooo much Force to swing the bat that it's pulling her front door out.
