Player Profile help

Does anyone have a sample or template of a player profile they've used and liked? Looking to create a one-pager of each player, saw some in the past but don't remember all the specifics they put on the document.

captain u and be recruited.com have examples. Most now use the on line profiles and have web pages posted so less paper and the college coaches can go on line an view them. Plus we post site to view the skills video's. Few use DVD now.
