As for plans, there are so many different approaches. I used to do a very regimented, time-tight practice where each skill was worked on by the whole team (think: basic infield drills and such), but this past season I started to transition over to a more games approach to my practices where the skills are practiced under pressure like with the below drill:

I start with walk-throughs and progressively increase the speed, and I'm less focussed on the time component, instead watching for skill mastery.
In regards to that first drill...I sure wouldn't want my team to practice hitting ground balls.
