runners on 2nd and 3rd 1 out.

Batter hits a ball off her foot but neither ump sees it to SS.

Runners take off and SS throws to 3rd to get runner who gets into a rundown and is tagged out after the runner on 3rd scores. In the meantime the batter things it foul so doesn't run and SS after tagging runner out throws to first for the 3rd out. Does the run count?

No, the run does not count as the 3rd out of the inning was the result of the BR not reaching 1B safely

No. No run can score when the third out is made on the batter-runner before they reach first base.

I think you are worried about too many extra things...ask yourself what would happen if SS fielded ball and tagged out the runner going to third, then the runner crossed the plate then the SS threw the runner out going to first. Double play no score

Right. It's not overly complicated, really.

Example: 1 out, R1 on third, R2 on second Fly ball to left which is caught for out 2. R2 tags and attempts to reach third while R1 leaves early and scores. They tag R2 out while attempting to get in to third after R1 has scored. Now there are three outs, but that run can still be nullified by a proper appeal on R1 leaving early. That's your "fourth out appeal". If not appealed, that run would count in this situation.

No, "fourth out appeal" only applies to a runner who has scored and nullifying his/her run.

This would be correct for ASA rules only.
All other rule sets I am aware of do allow for a fourth out appeal of a force play that would nullify a run, even if the runner being appealed did not score.
