As an aside, I've adopted the glove pointing down strategy (wrist points to target, fingers point down) as I've found personally that it decreases tension in my shoulders when pitching. That could be a quick fix, something to consider.

I tell the pitchers I work with to turn their glove hand inwards and point at their target with their little finger of their glove hand. It keeps the shoulder from turning out.

Start small and progress as long as she can maintain good reps. Use the slow motion feature on your iPhone (or whatever) for quality control. If something breaks down, change something. Try not to allow more than 2-3 bad reps before changing something. Decrease effort, motion or distance. Remind the pitcher that the person on the bucket is a fetcher, not a catcher. Throw into a net. Close the eyes. Learn to feel it. Learn to differentiate the good from the bad and make the correction. Own it.
