Unreported substituions

NFHS - girl goes into to play outfield unreported then comes up to bat. After first pitch a coach notices she is unreported. What is penalty? I believe it's just a warning. Please provide a cite so I can pass along.

Penalty
Players and substitutes shall not
enter the contest unreported. The
umpire shall issue a team warning to
the coach of the team involved and
the next offender on that team
shall
be restricted to the
dugout/bench for the remainder of
the game. The head coach is
also restricted to the dugout/bench
for the remainder of the game for a
second violation

In NFHS its just a warning to the coach as long as the girl is a legal sub. Next offense is coach and player restricted.

This happened once on a team we were playing. The umpire called her out. Later DH looked up the rule and found that it should've been just a warning.

The rule is different in USA softball. If discovered after a completed at bat, the batter is out and officially entered into the game. If discovered on defense and they made a play, the offensive coach gets the option of the result of the play or return runners and batter to bat and resume the original count and the player is entered into the game.

And to clarify. The unreported sub gets to stay in the game after the warning - you don't replace her with the original player after the warning?
