Avulsion fracture in pelvis

My 12 yr old daughter has been having pain in her drive leg hip area for about 5 weeks. At first we sorta blew it off as just being sore from doing pretty intense lessons working on really driving off that back leg and pulling the knee through. We went to an Airrosti Dr and he said that her muscles and tendons were extremely tight. We went a few times last week, then fast forward to this past weekend at a tournament and it was really giving her a lot of pain.

Long story short, we ended up getting an X-ray(which showed nothing) and an MRI. The MRI showed an Avulsion fracture of the superior iliac spine which is basically the tendons in her back leg pulling pieces of the growth plate in her pelvis away from the bone. It seems this is a common injury in sprinters and soccer players that have fast jerky movements using that muscle/tendon.

Has anybody had any experience with this injury and pitching?

We are going for a second opinion tomorrow, but basically the first doctor told us she will be out for at least 6 weeks. I understand rest and letting it heal, but my bigger concern is when she does go back to pitching, what is going to keep it from happening again? She had really been working hard the last couple months on exploding off the rubber(and I’m guessing ripping her bone off while doing it) and had actually gained about 4 mph in just a couple months. My daughter is pretty upset with the diagnosis as you can imagine being that she will be out for over half the season and not at her best for the remainder of the season when she comes back.

Anybody have experience with this?

I have nothing to offer thats useful other than prayers and well wishes, as I've never heard of your dd's condition!
Sounds awful and I wish you guys luck in a speedy recovery

Hope she has a full and speedy recovery. My DD had a back issue that she tried to work through. Finally found a great ortho who took care of it. He was our second opinion. Its good that you are seeking a second opinion. We thought our first doctor knew his stuff.

Best wishes for a complete recovery. I completely understand the concern and frustration over missing the playing and practice time but at the end of the day nothing is more important than the long term health of our kids. Exploding off the rubber is good but doing it repeatedly with possibly wrong, peripheral mechanics can certainly have other impacts. Good luck and keep the faith!! ;-)

Well that really stinks. I don’t have direct experience with this but fractures , even avulsion fractures take time to heal. She keeps going and it won’t heal and then you have a huge problem.
Prevention: I’m thinking 2 things. 1 . let’s hope her body matures and that helps. 2. Use a foam roller on those flexors to keep them pliable and less tight. Stretching should help as well. 3. You will need to modify her drills and exercise routines so she isn’t overworking those muscles. Clearly she did too much and the end result is a breakdown of tissue. Ie avulsion fracture. She just wasn’t ready for that much intense work.
Hope she has an uncomplicated recovery and a speedy one at that.

A girl we play with had this injury (not a pitcher though). She fully rested it (I believe 6 weeks also), and did physical therapy for a month to 6 weeks. I wish I could remember exact timing of everything, but long story short, after working PT and on her running form she came back stronger, faster, and has had no issues in the 1 year+ since then. Wish you well on this. My dd is coming back from an elbow injury and the best advice I can give is to do the rest/work correctly once so you don't have to do it again if possible. I promise that missing time now (at 12yo) is not as big a deal as it seems to be.

I have worked with two girls that had this when they came to me... one I shut down in her first lesson (3rd pitch) and sent them straight to the Dr... the other had a popping feeling in her hip, so we shut it down and had her go get tests. MRI is usually good, x-ray can be a good reference as a return-to-play check... as some of these injuries form a callus, I guess. With the first kid, she was out four months... but hers was pretty significant (I want to say it was that bone we sit on... ischial something or other). The latter sounds more like yours... and that was about 5-6 weeks.

I would be really curious... if you had any video of her motion prior to the injury? There are 3 areas that this is going to happen, sounds like it's during the initial explosion off the rubber.

Although not a doctor, by any means... I do have a bit of a trained eye regarding pitching movements... and might be able to help you guys isolate the issue... or at least offer some preventative ideas.

Totally up to you... but we all hope she has a speedy recovery!!! Best, ~JS
