Pitcher's setup in relation to middle of home plate

For a rhp, what part of the pitcher's body is over the middle of home plate? Right shoulder, right foot, belly button? Maybe you just line line up where the divot is in front of the mound?

I think the better questions are a) on which side of the "pitching plate" should the pitchers set-up and b) does it make a difference depending on the pitch. I don't think there has been much discussion on this topic. Would love to hear some of the experts thoughts on this?

My oldest has been to a number of instructors, including BH, and I have read most threads here over the past few years and very little has been mentioned about setup. I bring this up because my 10U constantly throws her fastball inside. Sure it could be a circle problem, or maybe her plant foot is too far to the right, but can't it be something as simple as moving over? My oldest had this same problem and refused to move because she thought that would be cheating tbe process. I didn't know much back then, so I agreed with her at the time.
 
My preference is that the pitcher sets up more to the throwing arm side of the pitching rubber. I still want them to stride straight out from there....which sets up some extra throwing/release angle plus it also is working with the natural IR/Pronation of the forearm and hand. So in this example a RHP's right foot would push from a spot about 4 inches from the right edge of the rubber and the landing foot toe will land about 4 inches from the right edge line of the rubber. Just the oppo for a LHP.
