If a team informs the umpire of a substitution but the umpire does not inform the opposing team is it a legal substitution? Is the game protestable if the ump allows the substitution?

NFHS - girl goes into to play outfield unreported then comes up to bat. After first pitch a coach notices she is unreported. What is penalty? I believe it's just a warning. Please provide a cite so I can pass along.
If my memory serves me I believe they changed the rule in travel ball where if you wrote the wrong number on the lineup card but the correct batter was batting there was no penalty.

Seems to me that the runner cut inside purposely to draw contact for obstruction (clearly she had no shot at a double). Is this clear-cut obstruction or can something like that be judged to be a no call or unsportsmanlike conduct?
